Apply for a Licence or Service

Please complete the application form with as much detail as possible to apply for a licence or a service for your church.

Unfortunately we cannot process your payment online - once we receive this form we will contact you to arrange payment.

With effect from 1 January 2011 the rate of VAT is being increased to 20%. Licence payments received in the New Year will be charged at the higher rate of VAT.
